REQUISITION
A lab requisition from your provider is required prior to your appointment. Please note that we do not provide lab orders. Your appointment will only be confirmed once we have received the requisition.

PAYMENT
Your invoice will arrive via email. It must be paid within 48 hours to confirm your appointment.

Cancellations
Please note that a 24-hour notice is required for all cancellations. If you fail to cancel within 24 hours of your scheduled appointment, a $75 cancellation fee will be charged. Thank you for your understanding.

Collection Hours:
Monday - Saturday 7:00am CT - 2:00pm CT
Please note that some laboratories do not accept collections on Saturdays. If this affects your appointment request, we will notify you and assist with rescheduling.

Additionally, our staff is trained to assist with any special needs or accommodations to make your visit as smooth as possible.
WHAT HAPPENS IF THE LAB DOES NOT ACCEPT THE SPECIMEN SAMPLE?
Client satisfaction is our highest priority. In the event a laboratory rejects a specimen due to an error attributable to our services, we will provide a recollection at no additional cost to the client.
